This form gathers important information including your medical history, current medications, lifestyle factors, treatment suitability, and informed consent. These questions allow us to assess safety, tailor your treatment to your skin and health profile, and ensure you achieve the best possible outcome from your HIFU treatment.

It is essential that this form is completed in full prior to your appointment, as treatment cannot proceed without it.

    Ensure and maintain sufficient hydration for at least 2 weeks prior.

  • Exfoliation

    Exfoliate your skin roughly 48 hours prior to the appointment time.

  • Corrective Actives

    Discontinue the use of Vitamin A, AHA or BHA products 72 hours prior to the treatment.

  • Sun Safety

    Ensure and maintain sufficient sun safety using preferably a Physical Zinc Sunscreen.
